5.1 Consolidating a community leadership hub for the health promotion action area.

Consolidate a leadership forum to provide guidance for, and lead, the health promotion action area. The forum is to be comprised of health promotion leaders – Primary Health Organisation (PHO) and community-based – and be ethnically diverse.

06/07 Operational Plan has increased funding for enhanced role for CODA to provide increased leadership in a number of areas

CODA meeting regularly and having positive input into the draft LBD operational plan.
Support and discussion for Healthy Kai project.
Discussion around screening for type 2 diabetes in the community.

Green Prescription Area Manager represents CMS on CODA.

Supporting activity:
MCC - Reorientation of Te Ora o Manukau/Manukau the Healthy City resources and priorities to support Tomorrow's Manukau/Manukau Apopo ‘Healthy People' goals and actions.

Child Advocacy Group - (Te Ora o Manukau-Manukau the Healthy City outcome group) has added obesity prevention and nutrition to work plan priorities.

 

5.2 Supporting aligned actions through better coordination of the funding environment.

Support funder organisations for health promotion in Counties Manukau to meet annually to share information on funding allocations and strategies as they relate to diabetes risk factors and disease management.

It is proposed LBD’s ‘life course’ model will be used to review funding strategies to minimise overlaps, identify gaps and ensure there are programmes operating in synergy with each other.

Develop over time a consistent and transparent approach to describing contacted outputs in summary terms so that providers can understand what each other is doing and who they are targeting.
The Qipps database and model provides an opportunity to support internet-based provider-to-provider sharing of strategic information.

Supporting activity:
MCC- LTCCP funding process and consultation opportunities reported to TM/MA Outcome Group. MCC 2007-2016 budgets for Advocacy and Programmes for Health (LTCCP) aligned with LBD objectives.

Draft revised organisational structure for MCC available for consultation. (To be confirmed by 1 April). Includes reunification of funding and service delivery and proposal for establishment of CCO for Recreation and Leisure Services.

Joint planning for Active Communities funding application - reports to Council committees to proceed with application (MCC, Franklin and Papakura)

 

5.3 Improving capacity of the health promotion workforce.

Develop an e-mail/web-based information network for health promoters working in Counties Manukau, that provides information on programmes, training opportunities, events, policies, practical issues (such as equipment and resource availability) and available specialist help in order to support an informed and interacting workforce that is focused on self directed improvement.

Commission the development of health promotion core competencies for diabetes risk factors and disease management. The training solutions may include services from independent providers and also the re-orientation of mainstream education service providers. Integrate formal health promotion competencies into career pathways.

Capacity development needs to have a focus on supporting Maori and Pacific health promoters.
Final competencies and report received. Developing a plan for implementation of these competencies.

Funding received from Ministry of Health for entry level training course in Health education and promotion

Funding received from Ministry of Health for mentoring and peer support for people who have attended Maori and Pacific train-the-trainer courses

Supporting activity:
MCC - MCC Health Promoting Schools Programme revised in consultation with principals and teacher-coordinator trainings days provided on identified health action priority areas.

Te Ora o Manukau - Manukau the Healthy City communications systems and planning revised.

Te Ora o Manukau - Manukau the Heatlhy City newsletter revised and aligned with MCC communications planning. Emphasis of promoting collaboration, advocacy and training opportunities.

MCC website revision project includes community/health wellbeing promotion activities of Council.

Professional development goals for Leisure Services staff to include REPS qualification.

Council support for ‘Getting Started' (Otara Health) Evaluation project to inform staff/service delivery monitoring.

5.4 Improving communications resources for diabetes for use within health promotion and primary care.

Undertake gap analysis for core diabetes communications resources for use in health promotion, obesity management and diabetes disease management. Gap analysis will look at issues of content, cultural applicability and quality. Health promotion leadership forum to oversee analysis process (multiple organisations have a stake in this process).

Resources will be updated or new resources developed, as required. Development and style of new resources should be cognizant of LBD social marketing strategies and of opportunities with electronic media, and the needs of LBD’s other action areas. Resources should also align with Patient Management Systems (where appropriate), the CCM programme and the New Zealand guidelines group.

Develop a well publicized and accessible supply of quality resources available for all health organisations, community organisations and groups and the wider community.

Contracted with Diabetes Projects Trust to complete resource suite development and delivery and promotion to users. ARPHS have agreed to stock resources for ongoing supply

Pamphlets and other resources for suites have been sourced (including paying for printing some resources)
